Job Description
- Client Assessment:
- Conduct comprehensive assessments of clients' needs, preferences, and goals.
- Work closely with clients to develop personalized support plans.
- Service Coordination:
- Identify and connect clients with appropriate support services, resources, and programs.
- Collaborate with service providers to ensure the delivery of high-quality and tailored support services.
- Documentation:
- Maintain accurate and up-to-date client records and documentation.
- Prepare reports on client progress and support plan outcomes.
- Communication:
- Communicate effectively with clients.
- Facilitate regular meetings to review and adjust support plans as needed.
