Position:Social Worker – WSIB Clinic

Department:Social Work and Patient Flow

Status: Part-time (0.6 FTE)

Hours: Monday to Sunday, days & evenings (hours subject to change based on operational needs of the organization)

Site: Tri-site

This is what you'll be doing:

As part of an interdisciplinary team, your primary focus is to support the workers. You will act as a resource to the interdisciplinary team through consultation and education as needed. You will provide information and facilitate connections to community resources based on the individual needs of the worker.

  • Conduct comprehensive psychosocial assessments, using a variety of modalities and in accordance with professional standards, clinical policies and practices.
  • Provide client centred psychotherapy in a group setting or one-to-one as indicated
  • In collaboration with professional colleagues, promote high standards of ethical clinical care to meet the workers needs
  • Participate in team conferences, interdisciplinary rounds, and educational rounds.
  • Intervene to alleviate high risk or vulnerable situations
  • Engage worker (and family) in problem-solving; enable goal-setting; assist with adjustment related to health issues (engaging, palliation, new diagnosis, etc.)
  • Formulate a plan of action to accomplish identified goals
  • Provide resource counselling though education and information as needed
  • Resolve complex issues with patient/family/SDM/POA through use of conflict resolution meetings, providing mediation and advocacy with both internal and external partners
  • Collaboration and information sharing with community partners within the “circle of care”
  • Appropriately escalate issues to Social Work and Patient Flow Manager and Director within expected timelines


This is what you'll need to do it:

  • BSW or MSW required.
  • Current registration and membership in good standing with the College of Social Workers and Social Service Workers
  • Minimum 3 years experience in a hospital or community health organization.
  • Strong clinical skills - assessment, counseling, and community integration.
  • Strong communication and collaborative skills.
  • CBT, DBT and Brief Solution Focused therapy experience preferred.
  • Familiarity with community resources, relevant consent and capacity legislation
  • Demonstrated ability to function independently.
  • Proactive in identifying worker needs.
  • Demonstrated ability to function effectively as a member of the health care team.
  • Strong ability to work collaboratively in a multidisciplinary team
  • Satisfactory employment and attendance record
  • Demonstrated ability to maintain an evidence-based practice approach.
